In addition, injuries acquired after the age of 40, tend to stay in some format and remain as a permanent pain the arse, or shoulder etc.<\/p>\n<p class=\"wp-block-paragraph\">The peer\u2011reviewed data on gym injuries is out there. I\u2019ve put it into tables and graphs to show the most common sites of injury:<\/p>\n<h2 class=\"wp-block-heading\">The Gym Injury League Table<\/h2>\n<p class=\"wp-block-paragraph\">Rank body parts by how often they get hurt in the gym and three names sit at the top of every study. Nobody swaps places.<\/p>\n<p class=\"wp-block-paragraph\">A Saudi cross\u2011sectional study of 1,012 gym\u2011goers gave the shoulder 25.2% of all injuries. The knee took 20.2%. The lower back took 17.7% (Hetaimish et al., 2024). Flip to a Dutch survey of 494 injured fitness participants. Same podium \u2013 shoulder, leg, knee (Kemler et al., 2022). Now look at a 25\u2011study systematic review of CrossFit covering 12,079 practitioners. Same three again: shoulder 26%, spine 24%, knee 18% (Rodr\u00edguez et al., 2022).<\/p>\n<p class=\"wp-block-paragraph\">Different sport. Different country. Same top 3 sites of injury (on the body).<\/p>\n<p class=\"wp-block-paragraph\">Here\u2019s how the whole league table looks when you smash the studies together:<\/p>\n<p>RankBody partShare of all gym injuries1Shoulder25 \u2013 26%2Lower back \/ spine17 \u2013 24%3Knee18 \u2013 20%4Elbow \/ wrist7 \u2013 10%5Hand \/ fingers5 \u2013 15%6Hip \/ hamstring3 \u2013 7%<\/p>\n<p class=\"wp-block-paragraph\">The shoulder is the delicate and injury-prone area. Doesn\u2019t matter what you do.  Lift, box, do CrossFit -the shoulder is first injury on the physio\u2019s table. There\u2019s a reason and it\u2019s boring: it has the widest range of motion in the human body, held together mostly by soft tissue. Load it hard, load it often, and it complains.<\/p>\n<h2 class=\"wp-block-heading\">The Hourly Wage of Pain<\/h2>\n<p class=\"wp-block-paragraph\">Counting total injuries is cute. Per\u2011hour rate is honest. A bodybuilder training six hours a week is not the same beast as a strongman training fifteen. Sports scientists get around this by counting injuries per 1,000 hours of training.<\/p>\n<p class=\"wp-block-paragraph\">Here\u2019s the leaderboard, drawn from the Keogh &amp; Winwood (2017) systematic review and the Rodr\u00edguez CrossFit review:<\/p>\n<p>ActivityInjuries per 1,000 hoursBodybuilding0.24 \u2013 1Powerlifting1 \u2013 4.4Olympic weightlifting2.4 \u2013 3.3CrossFit2 \u2013 5Strongman4.5 \u2013 6.1Highland Games7.5<\/p>\n<p class=\"wp-block-paragraph\">For context:<strong> football sits at 12 \u2013 35 injuries per 1,000 hours. Rugby is worse<\/strong>. So the gym is not a dangerous place. It just feels dangerous because injuries happen slowly, on your own, with heavy things nearby.<\/p>\n<p class=\"wp-block-paragraph\">Bodybuilding topping the safety chart will surprise a few people. It shouldn\u2019t. Isolation moves, machines, controlled tempo and never really testing a true max lift equals low injury rate. Trouble starts when you chase a PB.<\/p>\n<h2 class=\"wp-block-heading\">Who\u2019s Getting Hurt<\/h2>\n<p class=\"wp-block-paragraph\">The US emergency department data covers 970,801 weight\u2011training injuries between 1990 and 2007 (Kerr et al., 2010). Mean age 27.6. Men were 82.3% of cases. The upper trunk led at 25.3%, lower trunk second at 19.7%. That\u2019s a lot of tweaked backs.<\/p>\n<p class=\"wp-block-paragraph\">Gender plays a role. Men rack up more sprains and strains -the classic \u201cI loaded too much\u201d injury. Women pick up more accidental injuries -dropped plates, slips, catches. Men are roughly six times more likely to appear in the injury stats, which mostly reflects who\u2019s in the weight room rather than who\u2019s less coordinated (Quatman et al., 2009).<\/p>\n<p class=\"wp-block-paragraph\">Novices get hurt more per hour than experienced lifters. Competitors get hurt more than recreational lifters. The CrossFit review found a 40% injury rate in competitive practitioners versus 19.05% in recreational ones. Over double (Rodr\u00edguez et al., 2022).<\/p>\n<h2 class=\"wp-block-heading\">The Big Three Lifts \u2013 What Actually Goes Wrong<\/h2>\n<p class=\"wp-block-paragraph\"><strong>Bench press<\/strong> is where the single biggest chunk of gym injuries happens.<\/p>\n<p class=\"wp-block-paragraph\">In the Dutch data, 11.8% of all injuries came during a bench press (Kemler et al., 2022). Pec tears, shoulder pain and elbow strain top the list. A 2024 narrative review pinned most of the risk on muscular fatigue plus reps\u2011to\u2011failure plus grip\u2011width choices that stick the shoulder in an unstable position (Motlagh &amp; Lipps, 2024).<\/p>\n<p class=\"wp-block-paragraph\"><strong>Squat<\/strong> was behind 9.6% of injuries in the same Dutch data. Lower back and knees mostly. A 2025 systematic review of squat biomechanics showed that as intensity rises past 85% of 1RM, joint variability rises with it \u2013 the lifter\u2019s technique starts wobbling in ways that predict injury (Bakhshinejad et al., 2025).<\/p>\n<p class=\"wp-block-paragraph\"><strong>Deadlift<\/strong> doesn\u2019t show up nearly as often as you\u2019d think. But when it does, it\u2019s usually the lower back or a hamstring tear. And the mechanism is nearly always the same: rounded lower back at max effort.<\/p>\n<h2 class=\"wp-block-heading\">The Adjustments That Actually Cut Your Risk<\/h2>\n<p class=\"wp-block-paragraph\">The same studies that count injuries list the tweaks that stop them. This isn\u2019t guesswork.<\/p>\n<p class=\"wp-block-paragraph\"><strong>1. Warm up properly.<\/strong> Not treadmill\u2011and\u2011hope. A structured warm\u2011up cut injury rate by 36% across 15 pooled studies (Ding et al., 2022). Five to ten minutes of dynamic mobility, activation drills, and a couple of light\u2011load sets of the actual lift you\u2019re about to do.<\/p>\n<p class=\"wp-block-paragraph\"><strong>2. Cap your fatigue.<\/strong> Reps to failure is a shoulder injury waiting to happen. Velocity\u2011based training, RPE, or leaving 2 \u2013 3 reps in reserve give almost identical strength gains without the injury cost (Motlagh &amp; Lipps, 2024). Train hard. Don\u2019t train stupid.<\/p>\n<p class=\"wp-block-paragraph\"><strong>3. Progress load gradually.<\/strong> Big weekly jumps raise your risk. There\u2019s no perfect number \u2013 the classic \u201c10% a week\u201d rule was never actually tested in a proper trial -but the direction is obvious. Add small, add often. Not big and rare.<\/p>\n<p class=\"wp-block-paragraph\"><strong>4. Get supervised early on.<\/strong> Lack of coach supervision was a repeatable predictor of CrossFit injuries (Rodr\u00edguez et al., 2022). It\u2019s the same for general gym use. Seventy\u2011three percent of the Dutch fitness injuries happened during unsupervised training (Kemler et al., 2022). A few coached sessions at the start pay for themselves for a decade.<\/p>\n<p class=\"wp-block-paragraph\"><strong>5. Don\u2019t train hurt.<\/strong> Previous injury is the single strongest predictor of future injury in almost every gym\u2011injury paper on record. If it hurts, drop the load. Or drop the lift. Until it doesn\u2019t.<\/p>\n<p class=\"wp-block-paragraph\"><strong>6. Session length matters.<\/strong> The Saudi data showed sessions over two hours had higher rates of strains, dislocations and subluxations. Sessions of 1 \u2013 2 hours pumped up tendinopathy rates. Under an hour and you mostly got cramps and bruises (Hetaimish et al., 2024). If you\u2019re in the gym over two hours you\u2019re either an elite athlete or you\u2019re wasting time and stacking injury risk.<\/p>\n<p class=\"wp-block-paragraph\"><strong>7. Prehab the shoulder.<\/strong> Every study points at the same joint. Do one bit of prehab \u2013 banded external rotations and face pulls, two or three times a week. Five minutes. It targets the joint that ends more training blocks than anything else.<\/p>\n<h2 class=\"wp-block-heading\">Bottom Line<\/h2>\n<p class=\"wp-block-paragraph\">The gym is not dangerous. It\u2019s safer per hour than football, rugby, running or cycling. But the injuries you <em>do<\/em> pick up cluster in a very predictable pattern \u2013 shoulder, lower back, knee \u2013 and the causes are just as predictable. Too much load. Too fast. Too much fatigue. Too little warm\u2011up. Nobody watching.<\/p>\n<p class=\"wp-block-paragraph\">Fix those five things and you\u2019ll train for decades without a serious problem. Ignore them and you\u2019ll be reading this again after your first pec tear.
